We are out of network.


What that means is we do not work directly with insurance companies, but we can provide a monthly superbill for you to submit for reimbursement.


You may want to check your 

out of network benefits.


It is also important to know that a diagnosis must be given for insurance reimbursement. This may or may not be in your best interest.

 A consultation is required before an 

intake appointment and ongoing services 

can be scheduled.  


Technicians and interns are available at a significantly lower rate. Schedule a consult with Joshua or Michelle to ask if this is appropriate for you.


Joshua Moore is currently full. Please contact the office staff to be added to his service waitlists.


If you are scheduling an in-person consultation regarding a child 12 years old or younger, 

please do not bring your child to the initial consult.

How can I get a Superbill to request reimbursement from my insurance?

This is signed by the client to release any pertinent information to our office from another provider or vise versa. It may also need to be signed by an older teen who wants their parent to help with payments and scheduling.


If a client is over 14 years of age a release will need to be signed by the client in order to talk with parents.

Superbills are available upon request to current clients and accessible via your Client Portal. 


If a Superbill is requested by a parent of a client over 14 years old, a release of information needs to be signed by the client before the Superbill can be released.

Can I get an invoice for tax purposes?

General service costs are stated on the About Us page under each clinician. Costs are subject to vary depending on services rendered. 


Our rates range from $65 - $180 depending on the credentials and experience of the provider needed. This is also subject to availability.


Assessments are a separate cost.

We are happy to provide this for clients 

at their request.


If a tax receipt is requested for a client over 14 years old, a release of information needs to be signed by the client before the receipt can be released.
